Before an inmate's profile can be created on Prison Voices, they will need to fill out the application form. This asks them to write down all the information that will be needed by this website to create their profile.

I understand that you, as perhaps the inmate's mother, sister, brother, etc, might already know the information required, but they will be asked to write their own bio -- a couple paragraphs to describe themselves. Some people type out a bio for the inmate instead, and it comes out like, "This is my son, he's a good boy." And that really isn't a good idea. This gives them the chance to have their own voice, instead of having someone at home speak for them.

Print the Application

The application is available in Adobe Reader (.PDF) and Microsoft Word (.DOC) formats. When the application is loaded onto your screen, select the Print option from your browser's menu.

Adobe Reader: prisonvoices.pdf
Microsoft Word: prisonvoices.doc

Supplemental (Additional Info)

Microsoft Word: pv_supplemental.doc

Request a Copy to be Mailed

As a second option, you can have Prison Voices send a printed copy of the application to the inmate. Just fill out the form below. You must include your full name -- this will appear on the envelope so the inmate won't think it's unsolicited.
